I display a laminated copy of the CARE Vital Signs on a clipboard on a table in my waiting room with a note at the top in permanent purple marker, "Hello, please complete while waiting for your appt. Thank-you." Not everyone, but a significant number will do as I ask. At the bottom of the form, I've written, "Please take a HYH card" which is also on the table in its holder sitting on top of a stack of HYH books. When the pt walks into my office holding the card, it reminds me to mention they do the survey. I have found the care vitals sheet helpful in quickly identifying what is most concerning to the patient and it takes only a glance on my part to know what their issue is. A recent patient was in for refill of her levothyroxine but her health habit score was low so we addressed this issue as well. I have a patient with chronic back pain whose feeling score was high which led to a discussion his stressful marriage. Another pt with exacerbation of her BP had a high pain score due to her arthritis which was possibly also contributing to her HTN. I'm always looking for ways to get at what's really on patients' minds and this tool is definitely helpful.
